The prices are real. What the headline does not carry is the mechanism, and in drug pricing the mechanism is the medicine. The deal runs through a federal payment model called BALANCE, and participation is voluntary three times over: state Medicaid agencies choose whether to join, Part D plans choose, and the manufacturers choose [1]. A patient in a state that opts out gets the headline and not the price.
Then come the other two gates. Medicare coverage for obesity is conditioned on obesity plus at least one comorbidity - diabetes, heart disease, another qualifying diagnosis - so an obesity patient without one does not qualify [3].
