Case counts move, and they get the headlines. The number that determines what those counts do next is the vaccination rate, and it has crossed a line.

National kindergarten MMR coverage stands at 92.5 percent for the 2024-25 school year, down from 95.2 percent in 2019-20 [1]. The threshold generally needed to stop measles spreading through a community is about 95 percent [1].

The distance is 2.5 percentage points, and it is the difference between an introduced case burning out and an introduced case becoming an outbreak.

There is a second gap, between what is counted and what happens. Modeling of the Utah-Arizona border outbreak by Dr. Leisha Nolen indicates actual infections run three to four times larger than reported [1]. Applied backward, the 2025 Texas outbreak - 762 confirmed cases - may have been nearly twice that size [1]. The reported count is a floor, not a measurement.

The consequence has a date. In November, international health officials meet to determine whether the United States and Mexico have lost measles-free status [1] - an elimination the US achieved in 2000. Elimination is not a trophy; it is a statement that sustained transmission does not happen here, and it is decided on evidence.

'It's really critical, not only for individual people, but for communities, to have high vaccination coverage,' said Dr. Varun Shetty, Texas's chief state epidemiologist [1]. The community part is the mechanism. An infant too young for the vaccine is protected by the 95 percent, and by nothing else.
