Women who took estrogen-only menopausal hormone therapy were markedly less likely to be diagnosed with dementia, and their brains showed less Alzheimer's damage at autopsy, according to a Stanford Medicine study of 21,462 women. Published August 12 in Neurology, the journal of the American Academy of Neurology, the analysis found that estrogen-only users had 39% lower odds of a dementia diagnosis over their lifetime and 35% lower odds of Alzheimer's pathology when their brains were examined after death [1].
That autopsy piece is what sets the study apart. The researchers compared brain tissue from 258 women who had used estrogen-only therapy against 2,701 who had not, checking for the physical hallmarks of Alzheimer's rather than relying on a clinical diagnosis alone [1].
"MHT appears to exert a modest but meaningful and statistically significant protective effect on the risk of developing Alzheimer's pathology," said Jennifer Bruno, a study author [1].
The authors are explicit that this is a correlation. The association with reduced Alzheimer's risk is clear, said senior author Hadi Hosseini, but "it still doesn't amount to definitive proof of causality" [1].
The scope is narrow in ways that matter. The result applies only to the estrogen-only oral formulation, not to estrogen-plus-progestin therapy and not to topical estrogen [1]. It cannot rule out that women who took estrogen-only therapy were healthier or had different care to begin with, and it does not weigh hormone therapy's own known risks. What it adds is one of the most rigorous signals yet, grounded in actual brain tissue, that a widely used therapy may carry a benefit for the brain that is worth a real conversation between a woman and her doctor.
